Dr. Toby Meltzer, MD is a plastic and reconstructive surgeon who is globally recognized as one of the leading surgeons in the field of Gender Affirming Surgery. Dr. Meltzer has performed Top Surgery since the early '90s, making him one of the most experienced Top Surgery surgeons in the U.S. Dr. Meltzer offers Double Incision and Peri-Areolar Top Surgery in Arizona and Oregon.
Dr. Meltzer became the first surgeon in Arizona to specialize in Top Surgery when he moved from Portland to Scottsdale to expand his practice 2003. However, because Dr. Meltzer has always had a large number of patients based in the Pacific Northwest, he and Dr. Ley recently partnered with Legacy Good Samaritan to offer Top Surgery in Portland once again. Patients can choose between either Scottsdale or Portland for surgery dates.
Dr. Meltzer performs two types of Top Surgery: Double Incision with nipple grafts and Peri-Areolar.

Surgery Requirements
Dr. Meltzer follows WPATH guidelines for surgery. You will need to provide one referral from a qualified mental health professional recommending Top Surgery, unless otherwise specified by insurance.
Financing & Insurance
Dr. Meltzer accepts cash, credit cards and medical financing. He contracts with Kaiser and has also worked successfully with BCBS, UHC, Aetna, Cigna and Harvard Pilgrim insurance. Dr. Meltzer also accepts Medicaid (Apple Health.)
Booking a Consultation

NOTE: Dr. Meltzer is a regular presenter at transgender conferences, including First Event, Gender Spectrum, Gender Odyssey, Keystone, and Southern Comfort. Dr. Meltzer often does consultations at conferences, offering a convenient way for patients from around the country to meet with him without having to travel to Arizona or Oregon for a consult.
